Description
Funded by the Regional Permanency Resource Centers initiative of the New York State Office of Children and Family Services’ (OCFS), the Coalition opened the AGAPE Southern Tier-Finger Lakes office in 2017. In 2019 the program name changed to AGAPE Central New York and now serves Broome, Cayuga, Chemung, Cortland, Madison and Tompkins counties.
AGAPE Central New York serves families who have adopted a child or those that have become guardians of a relative’s child. By providing supportive services and connecting families to resources such as parent trainings, support groups for both parents and youths, peer mentoring, navigation for cross-system needs, therapeutic services referrals, counseling, or other supports to help families address issues as they arise, the AGAPE program helps post-adoptive and post-guardianship families through the unique challenges they face following an adoption or guardianship.
AGAPE’s services are provided free of charge. It is open for families who have adopted through foster care or private adoption whether domestically or internationally. Services are also available for those caring for a child through G docket guardianship or informal custody arrangements. Whether it’s a simple question, a need for more training or a more challenging problem, we are here to guide you.

We provide personalized one to one assistance, information & referral, community outreach, workshops and webinars for parents and professionals on a variety of topics relating to education, i.e., Special Education Laws from Section 504, EI to CSE ,Graduation and beyond. The Special Education Information Center assists parents and professionals to work in a collaborative manner to obtain appropriate information about the range of options, programs, services, and resources available to advocate effectively so that all children receive a free and appropriate public education in the Least Restrictive Environment. We are empowering parents so they can effectively advocate for their child/children in a very complicated services system.
In addition we offer an intensive Family Empowerment Advocacy Series one time per year. This 20-25 hour multi-session training series which prepares parents/guardians of children with disabilities to advocate within the educational system for their own children and as a volunteer for other families Participants are selected through an application process.

Description
Dolly Parton’s Imagination Library is a book gifting program that mails free, high-quality books to children from birth to age five, no matter their family’s income.
After launching in 1995, the program grew quickly. First books were only distributed to children living in Sevier County, Tennessee where Dolly grew up. It became such a success that in 2000 a national replication effort was underway. By 2003, Dolly Parton’s Imagination Library had mailed one million books. It would prove to be the first of many millions of books sent to children around the world.
Eligibility Requirements
There are no income or other qualifications to participate, and the program is available in several countries. Children can register and receive a free, age-appropriate book each month, mailed directly to their home.

Through Museums for All, those receiving food assistance (SNAP benefits) can gain free or reduced admission to more than 1,400 museums throughout the United States simply by presenting their SNAP EBT (Supplemental Nutrition Assistance Program Electronic Benefit Transfer) card. Find a participating museum near you or browse our full list of participating museums.
Museums for All is a national, branded access program that encourages individuals of all backgrounds to visit museums regularly and build lifelong museum habits. It is open to participation by any type of museum — including art, history, natural history/anthropology, and general museums, children’s museums, science centers, planetariums, nature centers, historic houses/sites, zoos, aquariums, botanical gardens, and arboretums.
The cost of museum admission can be a barrier for many low-income families. Participating museums provide reduced admission, ranging from free to $5.00, to visitors presenting their EBT card. This reduced rate is available during all normal operating hours to up to four individuals per EBT card. With a year-round open door policy, Museums for All invites low-income visitors to feel welcome at cultural institutions.

A Health Home is a community-based care management service model whereby all of an individual’s caregivers communicate with one another so that all social, physical, and behavioral needs are addressed in a comprehensive manner. This is done through a dedicated Care Manager who oversees and provides access to all of the services an individual needs to ensure they receive everything necessary to stay healthy, out of the emergency room and out of the hospital.
ELIGIBILITY
All children/youth must meet the following eligibility requirements to be considered for enrollment:
1.The child/youth is under the age of 21, enrolled in Medicaid and resides in a CHHUNY county.
2.The child/youth must meet the NYS DOH eligibility
3.Child/youth has significant behavioral, medical or social risk factors which can be addressed through care management.
